WAIVER STATEMENT
I assume full and complete responsibility for any injury or accident which may occur to me during the event, or while I am on the event course. I hereby release and hold harmless Forgotten Children International, sponsors, and all other persons and entities associated with the event. I further certify that I am in good physical condition and fully able to participate in this event. I understand that fees are not refundable and that bikes, skateboards, scooters, and skates are not allowed. Please type in your name if you agree. Entry is invalid if not signed (Parent/Guardian required if under 18)
